Please watch the video training and click on the button below to fill out the questionnaire. I suggest you either take notes, or fill out the questionnaire simultaneously in order to get all the answers for the questions. Once you submit the online questionnaire, I will know you have completed the training and update your volunteer file.

If you are 18 years old or older, you also need a valid police check. If you have not submitted one in the past 5 years, please click on the link below to submit one. You will need two pieces of ID and a credit card. The church will reimburse you for the cost, so please keep your receipt and send it to me. If online payment is not a possibility for you, please contact me as soon as possible so we can find and alternative solution.
